Choose the parts and assembly
Two-part forms commonly use white and canary; three-part adds pink, and four-part adds goldenrod. Five-part configurations and additional colors are available; ask for a sample before specifying a six-part set. Choose the sequence for your workflow rather than relying on a color to define its recipient.

Numbering and form layout
Choose a starting number and its position on the page. Sequential printing and crash numbering are different production methods; confirm which is appropriate for your set. Review the sequence, placement and numbering on every part in the proof or sample. Perforated tear-off stubs are also available.
Common layouts include invoices, work orders, receipts, estimates, delivery tickets and customer sign-offs. Use the space for the fields people actually complete: contact details, quantities, materials, labor and signatures. Supply your own approved terms.
For blank medical or dental forms, send unfilled templates without patient information. The practice supplies and approves the wording.

Bring an existing blank form or send a clear scan. Mark the fields, contact details or terms that need to change. For a new layout, describe the workflow and who receives each copy.
Design is $75/hour; a single-page layout is commonly estimated at 1–2 hours, with the actual work quoted first.

What is the minimum order?
The published minimum is 250 sets. Two-part pricing is $190 for 250, $250 for 500 and $320 for 1,000. Compare the quantities against how many forms you will use.
What do the add-ons cost?
Published add-ons are $45 for numbering, $59 for second-side printing and $65 for PMS ink matching. Confirm their application to your exact layout, part count and quantity in the quote.
Can you recreate our existing form?
Bring a blank sample or send a clear scan and mark the changes. We prepare a layout and proof for your review before printing.
How many copies can a form have?
Standard configurations use 2–5 parts. A six-part set needs a sample review to check the lower-copy readability for your intended use.
When will the forms be ready?
The usual estimate is 3–5 business days after approval, depending on quantity, part count and assembly. Call to check a rush request.
